Aboriginal Healthy Babies Healthy Children Coordinator

Position: Aboriginal Healthy Babies Healthy Children Coordinator
Start Date: ASAP following hire
Hours of Work: 32 hours per week

The successful candidate will be employed by the Fort Erie Native Cultural Centre Incorporated, responsible to the policies and directions determined by the Board of Directors and under the daily supervision of the Executive Director or their delegate.

Primary Objective

Ensure that urban Indigenous families and children (prenatal to age six) have access to effective and consistent early intervention services. Provide the best opportunities for healthy child development through home visiting, service coordination, parenting groups, cultural teachings, and traditions.

Education/Work Experience

  • Relevant college or university-level accreditation or equivalent work experience

Key Responsibilities and Duties

  • Conduct home visits to establish and develop a plan of care
  • Perform ongoing reviews of progress and support requirements for family support plans and make adjustments as needed
  • Identify additional supports or resources required, and assist with referrals, screening, and support services
  • Advocate on behalf of families and their children to ensure treatment is received in a culturally sensitive manner
  • Establish partnerships with professionals such as nurses, Elders, social service workers, midwives, Early Years staff, Brighter Futures, community health representatives, and community wellness workers
  • Initiate and participate in case management and case conferencing as necessary
  • Work to reduce and prevent family violence and improve Indigenous health by increasing awareness of specific issues, risks, or concerns in the group or the community
  • Foster healing and empower individuals, families, and the community to achieve and enjoy a healthy and balanced life
  • Work independently and in a team environment as a self-motivated individual
  • Exhibit exceptional oral and written communication skills

Knowledge

  • Take personal responsibility to increase sensitivity, awareness, and implementation of FENCC foundational teachings and organizational practices
  • Facilitate or coordinate programs or services involving traditional Indigenous knowledge, practices, medicines, teachings, or services
  • Implement traditional healing circles and cultural teaching circles such as medicinal teachings, Seven Grandfather teachings, and traditional parenting circles
  • Demonstrate high proficiency in Excel and other MS Office applications
  • Must have a current, clear, vulnerable sector police check
  • Must have a clean driver’s abstract and a current driver’s license
